Pharmaprix has announced their new Customer Care Survey,  enter to get a chance to win monthly prizes.

Entry to this contest is open only to residents of Canada who are at least the age of majority in the province or territory in which they reside.

To enter you first have to visit a Pharmaprix location and make a purchase, then visit the contest web site located at pharmaprixsondage.com and follow the on-screen instructions to fully complete and submit the online costumer survey.

This contest closes on December 31, 2020 at 11:59 pm ET.  Winners will be selected randomly from among all valid entries.
